Bioavailability is everything with curcumin — unformulated turmeric powder provides <1% absorption, making culinary use therapeutically irrelevant for systemic effects. Use bioenhanced formulations (Meriva, Theracurmin, or piperine combinations). Comparable to NSAIDs for OA pain in some trials. PIPERINE formulations inhibit CYP3A4 — major drug interaction concern for patients on medications [2] [3] [4].

3 Common questions
Why doesn't turmeric in food work like supplements?
Turmeric powder is only 2–5% curcumin, and unformulated curcumin has <1% bioavailability. A teaspoon of turmeric delivers <1 mg of systemically absorbed curcumin — far below therapeutic levels [2]. Bioenhanced supplements (Meriva, Theracurmin, piperine combinations) increase absorption 20–2,000× [3].
Is curcumin as effective as NSAIDs for joint pain?
Several RCTs have shown bioenhanced curcumin comparable to ibuprofen for osteoarthritis pain, with fewer GI side effects [4]. However, head-to-head comparisons are limited, and curcumin is slower to act (2–4 weeks). It is a reasonable alternative for mild-to-moderate OA pain.
Why does piperine matter?
Piperine (from black pepper) inhibits hepatic and intestinal glucuronidation of curcumin, increasing bioavailability ~20-fold [2]. HOWEVER, piperine also inhibits CYP3A4 and CYP2C9, which metabolize many drugs. Patients on medications metabolized by these enzymes should use piperine-free curcumin formulations (Meriva, Theracurmin) [2].
Can curcumin cure cancer?
Despite thousands of preclinical studies and enormous public interest, curcumin has NOT demonstrated anti-cancer efficacy in clinical trials [1]. The bioavailability problem makes systemic therapeutic levels nearly impossible to achieve. Some researchers have designated curcumin a 'PAINS' compound (pan-assay interference compound) — it interferes with many assay systems, potentially generating false-positive preclinical results [1].
4 Clinical evidence

Strong

In vitro and preclinical anti-inflammatory activity: curcumin inhibits NF-κB, COX-2, LOX-5, TNF-α, IL-1β, and IL-6 across hundreds of studies [1]. The bioavailability problem: unformulated curcumin achieves negligible plasma levels regardless of dose [2]. HIGH

Moderate

Osteoarthritis pain: meta-analysis of 8 RCTs showed curcumin (various formulations) significantly reduced pain and improved function, comparable to NSAIDs in some trials [4]. Ulcerative colitis (adjunctive): curcumin 1–3 g/day with mesalamine reduced relapse rates in an RCT [1]. Depressive symptoms: meta-analysis of 6 RCTs showed benefit of curcumin vs placebo [1]. Metabolic syndrome markers: modest improvements in fasting glucose, lipids, and inflammatory markers [1]. MODERATE

Insufficient

Cancer treatment or prevention: despite extensive preclinical data, clinical trials have not demonstrated benefit, largely due to bioavailability limitations [1]. 6 Interactions

Drug interactions

CYP3A4/CYP2C9-metabolized drugs (via piperine) Major
Mechanism: PIPERINE (not curcumin itself) inhibits CYP3A4, CYP2C9, CYP2C19, and P-glycoprotein. This dramatically increases blood levels of drugs metabolized by these enzymes. [2]
Effect: Elevated blood levels of: statins, cyclosporine, tacrolimus, warfarin, phenytoin, theophylline, and many others. Potential toxicity. [2]
Action: For patients on medications: use piperine-FREE curcumin formulations (Meriva, Theracurmin). If using piperine, be aware of all CYP interactions [2].
Anticoagulants and antiplatelets Moderate
Mechanism: Curcumin inhibits platelet aggregation and thromboxane synthesis. [1]
Effect: Additive bleeding risk. [1]
Action: Monitor for bleeding symptoms. Use caution at doses >1 g/day curcuminoids with anticoagulants [1].

Boswellia (frankincense) · 300–500 mg boswellic acids/day
Curcumin + boswellia (5-LOX inhibitor) provides dual anti-inflammatory action. Meta-analysis supports the combination for OA [4].
