Can zinc supplements cause copper deficiency? ▼
Yes — this is the most common cause of acquired copper deficiency in developed countries. Zinc induces metallothionein in intestinal cells, which traps copper and prevents absorption. This occurs at zinc doses >40 mg/day for weeks to months [3]. Denture cream containing excess zinc has caused severe copper deficiency with myelopathy.
How is copper deficiency different from iron deficiency? ▼
Both cause microcytic anemia, but copper deficiency also causes neutropenia (not seen in iron deficiency) and does NOT respond to iron supplementation. Copper is required for ceruloplasmin, which mobilizes iron from stores [2]. If an anemia is refractory to iron, check copper levels.
What is Wilson disease? ▼
Wilson disease is an autosomal recessive disorder (ATP7B mutation) causing copper accumulation in liver and brain. It presents with liver disease (ages 5–35), neurological dysfunction, and Kayser-Fleischer rings. Treatment includes copper chelators (penicillamine, trientine) and zinc (to block absorption). Untreated, it is fatal [3].
