⚠ Patient using ear oil with perforated eardrum — contraindicated [1]
⚠ Patient smoking mullein for lung health — paradoxical; use oral forms [1]
⚠ Practitioner citing mullein RCTs — there are essentially none for mullein alone [1]
ℹ️ Not obtained from food. Not applicable — this is not obtained from food in meaningful amounts; supplementation is the practical route.
🔬 Lab interpreter
ℹ️ No validated blood test. There is no established laboratory test to assess status or guide dosing for this ingredient. Clinical response and symptoms are the practical guide.

Clinical verdict
Mullein is the most widely used respiratory demulcent with essentially zero RCT evidence — its reputation rests on centuries of clinical experience and plausible pharmacology (mucilage + saponin expectorant). Two practical teaching points: (1) ALWAYS strain tea through fine cloth (trichome irritation); (2) mullein ear oil is a combination product in the only RCT — mullein's isolated effect is unknown [1].

⚖️ Obesity: Standard dose
Fat-soluble compounds may require dose adjustment in obesity.
🩺 Renal: Consult specialist
Dose adjustment may be needed in renal impairment.

How to take
🍽 Timing: TID for respiratory complaints [1].
💊 With food: Tea on its own or with honey [1].
🚫 Avoid: Unfiltered tea (trichomes). Ear drops with perforated eardrum. Smoking mullein [1].
2 Which form?
| Form | Bioavailability | Vegan | Cost |

| ['Mullein leaf tincture', 'convenient', '1–4 mL TID. Alcohol extract. Concentrated [1].'] | Standard | Check label | |
| ['Mullein leaf capsules', 'convenient', '500–1,000 mg TID [1].'] | Standard | Check label | |
| ['Mullein flower ear oil', 'topical', 'Mullein flowers infused in olive oil. Warm drops in affected ear. Traditional otitis media remedy [1].'] | Standard | Check label |
3 Common questions
Why strain mullein tea through cloth? ▼
Mullein leaves are covered in tiny hair-like trichomes that can irritate the throat and mucous membranes — exactly the opposite of what you want from a cough remedy. Always strain through fine muslin or a coffee filter to remove these hairs [1].
Does mullein ear oil actually work? ▼
One RCT found a herbal combination ear oil (including mullein) effective for ear pain, but mullein's specific contribution is unknown. The traditional practice of warm oil drops may itself provide pain relief via thermal comfort. Do not use ear drops if the eardrum is perforated [1].
Why is mullein recommended if there's no RCT evidence? ▼
Mullein's demulcent (mucilage coating) and expectorant (saponin-mediated mucus thinning) mechanisms are pharmacologically plausible and well-characterized. The lack of RCTs reflects insufficient commercial interest in studying a common weed, not evidence of inefficacy. Clinical experience over centuries supports its role for mild respiratory complaints [1].
Is mullein safe to smoke? ▼
Some herbalists recommend smoking dried mullein leaf for respiratory conditions — this is paradoxical and not recommended. Inhaling any combusted plant material introduces carcinogens and irritants. Use tea, tincture, or capsule forms [1].
4 Clinical evidence
Strong
No RCTs or systematic reviews for mullein alone [1]. HIGH
Moderate
Ear infections: a combination ear oil (mullein + garlic + St. John's wort + calendula) was as effective as anesthetic ear drops in 1 RCT (n=103) for otitis media pain — but mullein's individual contribution cannot be isolated [1]. Antibacterial: mullein extracts show activity against S. aureus, E. coli, and K. pneumoniae in vitro [1]. MODERATE
Insufficient
5 Safety, toxicity & adverse events
Relative
⚠ Filter teas well — the plant hairs can irritate the throat
⚠ Sedatives — possible additive drowsiness
⚠ Pregnancy and lactation — limited data
🚩 Red flags
● Patient with persistent cough >3 weeks using mullein as sole treatment — needs medical evaluation [1]
● Ear drops in perforated eardrum [1]
6 Interactions
