CONTRAINDICATED in oestrogen receptor-positive (ER+) breast cancer, endometrial cancer, uterine fibroids, endometriosis — phytooestrogenic components may stimulate oestrogen-dependent pathways. Drug interactions: tamoxifen, aromatase inhibitors, HRT. Hepatotoxicity reported with black cohosh. Pregnancy: CONTRAINDICATED. Requires individualised risk-benefit assessment.
