TMAO conversion: gut bacteria metabolise carnitine to trimethylamine (TMA) then TMAO — elevated TMAO associated with cardiovascular risk. High-dose L-carnitine supplementation may increase TMAO. Drug interactions: thyroid medications (reduces thyroid hormone activity), valproate (may increase L-carnitine clearance), warfarin (potential interaction).
