Reduced activated folate form. Used as leucovorin rescue in methotrexate therapy and as active folate supplement. Bypasses dihydrofolate reductase.

Leucovorin rescue: prescribed in methotrexate toxicity — distinguish from supplement use. Active folate — same general considerations as 5-MTHF. Does not mask B12 deficiency like folic acid. Drug interaction: methotrexate (used to rescue).
